In accordance with your contract, this form will serve as the “written request” to cancel your AMS Services. This is a request only and you will receive confirmation via YM’s billing department once your current contract has been reviewed and your request has been approved. You will also receive any additional information via email from our billing team with details on your terms and contractual obligations tied to your current agreement with YM.
